What is alopecia areata barbae?
Alopecia areata barbae is hair loss as patches in the beard area.
Alopecia areata barbae is a non contagious condition affecting a very small percentage of men. It has no specific cause and there is no standard treatment. It is considered to be an autoimmune disorder. The immune mechanism mistakenly identifies the hair follicle as an antigen and attacks it. This autoimmune activity leads to inflammation of the hair follicles and the detachment of the affected hair. Hereditary predisposition is also a major contributing factor for developing Alopecia areata barbae.

Those men suffering from spotted hair loss on the scalp may or may not develop A. areata barbae. The disorder can also affect apparently perfectly healthy men having no problem on the scalp. In some cases the disorder may resolve by itself and disappear. In some men AA. barbae may recur. The affected area may remain as a patch or progress to cover the entire beard. In rare cases there may be involvement of nails like pitting, thinning or detachment from the nail bed. In rare cases burning, itching or tingling sensation is experienced in the affected skin.

Causes of alopecia areata barbae

Though it known that autoimmune reactions are the cause of hair loss, it is not still clearly understood why the hair loss appears as patches on the scalp and/or beard area and why particularly certain areas are only being targeted.

It also not clear why perifollicular accumulation of antigen-presenting cells occurs at the site of AA. barbae. Hormonal imbalances, extreme mental or physical stress, environmental stress, allergic reactions, chemicals and presence of autoimmune diseases may contribute or trigger AA. barbae disorder.

Diagnosis and differential diagnosis

Differential diagnosis has to be done to rule out certain conditions wherein hair loss occurs in the beard area. Infection or inflammation of the follicles as in tinea barbae and folliculitis barbae may sometimes cause temporary or permanent damage to hair follicles and the affected area may appear as hairless patch after resolution of the condition.

Pseudofolliculitis barbae (shave bumps) affected skin may also show patches of hair loss in the affected area after the resolution. In such conditions the observed hair loss on the beard area is not alopecia areata barbae.

Treatment options

Early diagnosis and treatment can prevent aggravation and mental stress. Shaving daily is the immediate treatment. Immunosuppression of cytokines responsible for the inflammatory reactions is the best treatment option. Immunosuppression causes inhibitory effect on the autoimmunity at the site of AA. barbae.

Intralesional corticosteroid injections or topical corticosteroids shows good results but prolonged use can lead to thickening or thinning of the skin. Minoxidil, an antihypertensive vasodilator medication is found to promote regrowth of hair. Anthralin, a synthetic anti-psoriatic medication controls progress of alopecia areata barbae and promotes hair growth.

What is alopecia areata totalis?
Totalis is an extensive form of alopecia areata (AA). Alopecia areata totalis causes complete loss of hair on the scalp.
AA. totalis is an autoimmune disorder wherein the body's immune mechanism mistakenly considers the hair follicles on the scalp as harmful foreign bodies and attacks them. It is a non contagious condition with no specific cause and no standard treatment. Nearly 5% of the persons affected by alopecia areata may have complete hair loss on the scalp and develop the totalis form.

Though it can affect people of all ages, young children and young adults are more prone. Young children affected by hair loss may develop AA. totalis condition when they grow up. From the start of the symptoms, it may take less than six months for losing all the scalp hair. It is also seen that the genetic predisposition is a major cause and contributing factor for developing AA. totalis. In many cases the problem may cause psychological effects such as depression and emotional distress.

The symptoms first appear as patches of hair loss on the scalp. It progresses rapidly and spreads to entire scalp.

Causes of alopecia areata totalis

AA. totalis is believed to be T cell-mediated autoimmune disease affecting the hair follicles. Antigen-presenting dendritic cells, CD4+ helper T cells, cytotoxic CD8+ T cells and a few polymorphonuclear leukocytes are seen as perifollicular accumulation at the site of AA.totalis. Human leukocyte antigen DQ3 (DQB1*03), HLA DQ7 (DQB1*0301) and HLA DR4 (DRB1*0401) are often detected in AA. totalis and universalis patients.

Treatment of alopecia areata totalis

Immunosuppression of cytokines, Interleukin 2 (IL-2), Interleukin 1 alpha (IL-1 alpha) and interleukin 1 beta (IL-1 beta) which participate in the regulation of immune responses, inflammatory reactions and hematopoiesis, causes inhibitory effect on the development of AA. totalis.

Topical steroids and intralesional steroid injections have proven to be unreliable treatments. Systemic steroids and PUVA ultraviolet light treatment have given mixed results. Immunotherapy has been the most successful treatment for AA. totalis. By applying low concentration of an allergen on the scalp, contact allergic dermatitis is induced. The resultant cascade of immunomodulation and immunosuppression lead to suppression of autoimmune activity and causes regrowth of hair.

Turban PUVASOL therapy?

L Sornakumar et al of Department of Dermatology, PSG Hospitals, Coimbatore, India, have reported two cases of AA. totalis treated with "turban psoralen with sunlight exposure (PUVASOL)". A solution of 1 ml of 8-methoxypsoralen (8-MOP) 1% diluted in 2 liters of water was prepared. A clean absorbent cotton cloth was soaked into the solution and then wrapped around the scalp for 5 minutes.

The scalp application was repeated four times and later the patient was exposed to sunlight for 5-15 minutes. In six months there was almost complete regrowth of scalp hair. The method did not cause any systemic side effects encountered in PUVA treatment.

What is alopecia areata universalis?
Universalis is an extreme type of alopecia areata (AA).
A. areata universalis causes complete loss of hair over the entire scalp, face and body. A.areata universalis is considered to be an autoimmune disorder. It may occur at any age in both the genders. In about 60% of cases, the first initiation of hair loss may occur before 20 years. There is no specific cause for alopecia areata universalis disorder and there is no standard treatment. Complete regrowth is possible without any treatment. Some persons may have repeated cycles of hair loss and regrowth. Approximately 1 in 100,000 of the population is affected by alopecia areata universalis.

As in many autoimmune diseases, some unknown factors trigger and cause the immune system to mistakenly attack hair follicles and arrest the growth of hair. Fortunately, the hair follicle stem cells remain alive and with appropriate immune signal the normal hair growth may resume. It may take a few months to a few years to outgrow the AA. universalis disorder and resume the natural regrowth. In some unfortunate persons regrowth may never occur. AA. universalis is often psychologically devastating with severe social and emotional impact.

Causes of alopecia areata universalis

Alopecia areata universalis is not contagious and is considered to be an autoimmune disorder.
  • It is caused by the body's immune system considering its own hair follicles as foreign bodies and attacking them. However the stem cells contributing to the development of hair follicles are left unharmed.

    As in most of the autoimmune diseases, hereditary predisposition is a major contributing cause of AA. universalis. Angela M. Christiano et al of Columbia University Medical Center uncovered eight genes responsible for the onset of the hair loss disorder. Gene ULBP3 and two other genes are expressed in hair follicle attracting cells marked by a killer cell receptor, known as NKG2D. Other five genes are connected with immune response.

    The differences in causes for individuals to lose hair only as circumscribed patches on the head or to lose totally on the head and body are not clearly known.
  • Hormones, extreme stress, allergies, environmental factors, presence of autoimmune diseases, viruses, chemicals, toxins, infection and medicines might cause, contribute or trigger AA. universalis in genetically predisposed individuals.

    Alopecia areata universalis may also cause extrafollicular involvement in nails as pitting, koilonychias and onycholysis. AA.universalis is also known to cause ocular changes such as lens opacities, decrease in visual acuity and cataracts.

    Treatment options for alopecia areata universalis

    Many AA. universalis treatment options are available. The results are not consistent and show varying levels of success among individuals.
  • Extensive hair loss makes the treatment prolonged and difficult. As earlier said the stem cells of hair follicles are unaffected by the immune system attack. This leaves a window open for the regrowth of the hair at a later date when the particular autoimmunity wanes or triggering factors withdraw. In some AA. Universalis patients, the regrowth may start naturally without any treatment. Camouflaging hair loss with a wig is the best solution in persisting AA. universalis. We may consider a few of the treatment options available.
    Cortisone injections, creams and pills:
    Topical corticosteroids and intralesional corticosteroid injections show good results in patients with patchy hair loss. However AA.universalis responds poorly to intralesional corticosteroid injections and topical corticosteroids. Systemic corticosteroid pills have been found to be effective in the treatment of AA.universalis.
    Topical Minoxidil:
    Minoxidil stimulates proliferation and differentiation of the keratinocytes and is helpful in persons with patchy hair loss. However in patients with AA. universalis the regrowth is poor.
    Dithranol:
    Dithranol has immunosuppressant and anti-inflammatory properties. In some patients with AA. universalis, dithranol has been observed to cause positive response. Its use requires further studies and standardization.
    Topical immunotherapy:
    Topical immunotherapy have been found to be effective in the treatment of alopecia areata universalis. In this procedure allergic contact dermatitis is induced by applying a potent contact allergen. The contact allergen is applied periodically to cause sensitization. The sensitization of the scalp brings about immunomodulatory actions which inhibit the autoimmune reaction against the hair follicle. 50-60% of alopecia areata universalis patients undergoing topical immunotherapy treatment achieve a worthwhile response.
    Immunosuppressive agents:
    Immunosuppressive agents azathioprine, cyclosporine and sulfasalazine have been tried in the treatment of AA. universalis with positive results. However these medicines cause side effects such as gastrointestinal distress and liver toxicity.
    Tofacitinib citrate
    Brittany G Craiglow et al in their trial on a person with both plaque psoriasis and alopecia areata universalis, administered tofacitinib citrate, a rheumatoid arthritis drug. The patient grew a full head of hair, facial hair and body hair. The medication was mildly effective on psoriasis. Further studies are being carried out on this novel treatment for alopecia areata universalis.

    What is alopecia areata?
    Alopecia areata is the medical term for sudden transient loss of hair in circumscribed patches especially of the scalp or face.
    Initially alopecia areata patches may appear round or oval with the marginal broken hairs having a typical exclamation point or '!' appearance. Though the exact cause is not clear, research evidence suggest an autoimmune origin.

    This disorder is also found to run in families and affects 1.7% of the population at some point in their lives. Alopecia areata is also found to be associated with some autoimmune diseases and hereditary disorders. Intense emotional state of stress, anxiety and panic are common triggering and precipitating factors.

    Both men and women may be affected by this type of alopecia. In men beard area may also be involved. Children and young adults are affected more often than other age groups. Spontaneous remission and recurrence of alopecia areata is common. In some cases the condition may be progressive and result in total hair loss on the head and body.

    What are the types of alopecia areata?

    These patches of hair loss are categorized into subtypes considering the extent of hair loss as well as the affected areas.


  • In some individuals the hairless patches may grow leading to total loss of hair on the scalp in about six months. The condition may become permanent is some individuals whereas in some hair may grow back completely.


    In AA universalis, hair loss may spread to entire epidermis causing total loss of scalp and body hair. The condition may become permanent is some individuals whereas in some hair may grow back completely.

    Alopecia incognita (diffuse alopecia areata)
    AA incognita is a very rare type of hair loss with rapid progress, found mostly in young women. The disorder is clinically different from other types as it is diffuse and is lacking in well-defined patches.

    Noninflammatory hair loss in sharply defined areas on the chin is known as alopecia barbae. These patches may appear along with patches on the scalp or appear only on the beard area.

    What are alopecia areata causes?

  • In AA with autoimmune origin the body's immune system mistakenly attacks the hair follicles causing hair loss. There is association of the disorder with other autoimmune diseases like autoimmune thyroiditis, diabetes mellitus, vitiligo, rheumatoid arthritis and psoriasis. Some people have the disorder running in their families. In such cases having genetic predisposition certain triggers like severe viral infection, severe stress, panic or anxiety may precipitate hair loss. Certain environmental factors may also play a major role in the etiopathogenesis. Nutritional deficiency, especially iron or zinc deficiency is reported to precipitate the hair loss.

    What are alopecia areata symptoms?

    Sudden loss of hair in round or oval patches is the only symptom. Usually, the hair loss occurs on the scalp. It may also occur on the hairy parts of the body. In very rare cases well-defined linear hair loss is seen. Broken hairs at the edge of the patch may look like exclamation points. In rare instances burning sensation or itching had been reported. Nail pitting, multiple leukonychia and trachyonychia are observed in 30-50% of the cases of alopecia areata.

    Alopecia areata treatment and management

    There is no cure for this disorder. Corticosteroids are the preferred medication in the treatment of this disorder. They may be administered in oral, topical or intralesional preparations.
    Minoxidil, an antihypertensive vasodilator medication, is found to arrest hair loss and stimulate hair growth.
    Anthralin (dithranol) a synthetic oxidative immunosuppressive compound with anti-psoriatic action is used successfully with short contact treatment. It may have side effects like severe irritation and folliculitis.
    Other forms of treatment for alopecia areata include topical immunotherapy, phototherapy, use of prostaglandin analogues, use of topical calcineurin inhibitors, Sulfasalazine use and mesotherapy.

    Alopecia is loss of hair from the head and body. Alopecia can be broadly grouped into familial (genetic) type or acquired type.
    Androgenetic Alopecia and its subgroups come under genetic types. The androgenetic hair loss tend to run in families. Alopecia areata, an autoimmune disorder, appear to have genetic predisposition in some cases.

    Acquired baldness encompasses all other types of hair loss. Acquired alopecia is further categorized with respect to the causative factors and the extent of affliction. The triggering factors include certain medical conditions, diseases, febrile illness, psychological disorders, chemicals, cancer chemotherapy, parturition and habits. Certain types of alopecia disappear on their own whereas some types have no cure. Identifying the type of hair loss is essential for an effective treatment.

    Androgenetic Alopecia

    Androgenetic alopecia is a common type of androgen induced hair loss disorder affecting genetically predisposed men and women. The genetic predisposition and the presence of increased androgen gradually convert terminal hair into short, fine, light-colored, and barely noticeable . Androgenic hormones have different effect on follicles at different areas of the body.

    Androgens stimulate growth of terminal hair on the face, armpits and pubic area on reaching puberty.     Androgenic loss of hair in men begins above the temples, frontal hairline and vertex. With advancing age a rim of hair at the sides and rear of the head remains. In some rare cases it can progress into total loss of hair. Though loss of hair may cause psychological stress due to its effect on appearance, men are able to cope and retain integrity of personality. In Hamilton–Norwood scale of classification of hair loss in men, the progression is graded from type I to VII.
  • Dr. James Hamilton introduced the measurement scale in 1951 and it was later revised and updated by Dr O'Tar Norwood in the 1975. Hamilton-Norwood scale is frequently used for clinically assessing the severity of androgenetic alopecia in men.
    Female pattern hair loss
    Female pattern hair loss is becoming more common and the prevalence increases with advancing age. In female pattern there is thinning of hair all over the head. However unlike the male pattern, the frontal hairline does not recede. Further, in female pattern very rarely there is total loss of hair. Female pattern is considered androgenetic type if there is androgen increase. Affected women suffer psychological distress and their social life is impaired. In Ludwig's classification of hair loss in women, the progression is graded as minimal (type I), moderate (type II), and severe (type III).

    Alopecia areata

    Alopecia areata typically appears as small round patches of hair loss with underlying smooth unscarred skin. It is a non contagious disorder believed to be caused by autoimmunity. The body's immune system attacks its own anagen hair follicles, suppressing or stopping hair growth in the affected area of skin. This type of loss of hair can appear on scalp, beard or any other hair bearing part of the body. In many cases the hair grows back after a few months or years. In some rare cases hair loss may progress to cause total loss of hair on the head or total loss of hair all over the body including head.
    Alopecia totalis
    (AAT) is a rare severe hair loss. In this type there is total loss of hair on the head. Autoimmunity is considered to be the cause of this disorder. In many cases the hair regrows. Methotrexate, an antimetabolite and antifolate drug used for some autoimmune diseases and cancer may suppress the disorder.
    Alopecia universalis
    (AAU) is also a rare and severe hair loss. In this type hair fall is rapid and there is total hair loss from the body including head, eyelashes and eyebrows. The condition may appear at any age and in both the genders. The condition may resolve fast and the hair may start regrowing. In some individuals the regrowth may start after several years and in some it may not regrow at all. AAU is believed to be a autoimmune disorder.
    Alopecia barbae
    This is a condition affecting only men wherein alopecia areata is restricted to the beard. This is again an autoimmune condition wherein the body's immune mechanism mistakenly destroys or suppresses the hair follicles.

    Alopecia mucinosa

    Alopecia mucinosa (follicular mucinosis) is rare skin disorder with characteristic hairless scaly pink, flat patches without hair on the scalp and face. When observed under microscope, mucin is seen deposited around hair follicles. The exact cause of alopecia mucinosa is not known and cell mediated immunity may be the causative factor.

    Anagen effluvium

    Anagen effluvium is loss of hair under growth phase.
  • Primarily This type of alopecia is caused by chemotherapy and radiation therapy. In most of the individuals the hair grows after the cessation of the therapy.

    Telogen effluvium

    Telogen effluvium is the loss of hair during the telogen phase (hair follicle resting phase). Usually emotional or physical stress is the cause of this type of alopecia. prolonged febrile illness, childbirth, major surgery, chemicals and drugs can be the causative factors.

    Scarring Alopecia

    Cicatricial alopecia (aka scarring alopecia) is a group of hair loss conditions wherein the hair follicles are destroyed and replaced by scar tissue. The scar may be apparent or invisible and deeper inside the skin. The hair loss is permanent. This type of alopecia can affect both men and women. The causes are inflammatory diseases affecting the hair follicles where the stem cells and sebaceous glands are located. With the destruction of stem cells and sebaceous glands there is no regeneration of follicles. Hair follicle destructive inflammatory process and external injuries such as severe scalp infections, burns, radiation, tumors, and traction are some of the causative factors.

    Traction Alopecia

    Traction Alopecia is a type of alopecia caused by application of pulling force. Wearing tight pigtails, ponytail or braids causes the hairline to recede due to constant hair pulling. Extended use of hairpiece, wig, headgear or tightly fitting safety helmet may also cause this type of alopecia.

    Trichotillomania

    Trichotillomania is hair pulling disorder. The affected individuals have a compulsive urge to pull out their own hair. In this type of alopecia patches of hair loss occur. It is a psychological disorder and may get triggered by stress and depression. Affected individuals have tendency to keep pulling hair from head, eyelashes, eyebrows, nose and hands. Most of the patients with trichotillomania do not realize that they are pulling their hair. If the disorder is present in childhood, the child may outgrow the habit. If this type of habit persists a psychiatrist may be consulted for behavioral evaluation and treatment.

    Pressure alopecia

    Pressure alopecia is often seen in infants and bedridden patients. Lying in the same position and constant pressure on the rear of the head causes alopecia.

    Alopecia means generalized hair loss as patches or total in hairy parts of the body such as head, chin, eyebrows or eyelashes. Unlike alopecia, baldness is specific to head and is usually defined as the condition of having no hair on the top of the head. The pattern of hairlessness may be focal, multifocal, symmetric, or generalized.

    What are the types of alopecia?

    The condition can be broadly categorized into genetic and acquired types. Types with genetic predisposition are:
    • Androgenetic alopecia is male pattern and female pattern baldness.
    • Alopecia areata is a condition caused by autoimmune disorders.
    • A. universalis is total hairlessness on the head, eyebrows, eyelashes and body.
    • In a. totalis there is total baldness of the head.
    Acquired baldness encompasses all other types of hairlessness:
    Hair loss
    • Trichotillomania is a psychological disorder in which the patient develops the compulsive urge to pull out his own hair.
    • Telogen effluvium is rapid excessive hair loss brought about by stress factors such as parturition, febrile illness, major surgery, poisoning, and psychological stress.
    • Anagen effluvium is the loss of the growth phase, usually brought about by cancer chemotherapy.

    What are alopecia symptoms?

  • There are several symptoms denoting the presence of this problem. Early symptoms include unusual thinning and shedding while brushing and washing. Appearance of bald patches, thinning of eyebrows, thinning of eyelashes, presence of dandruff, skin lesions and skin scarring are some of the signs and symptoms.

    What are the causes of alopecia?

    Causes of hairlessness are not completely understood and can be caused by several factors.
    Anorexia nervosa,
    autoimmune disorder,
    burns,
    fungal infection,
    chemotherapy,
    chronic debilitating illness,
    extreme physical stress,
    genetic predisposition,
    heavy metal poisoning,
    hormonal effects,
    thyroid diseases,
    nutritional deficiencies,
    parturition,
    psychological disorder,
    psychological stress,
    radiotherapy,
    sudden blood loss,
    seborrheic dermatitis and
    traumatic damage are some of the known triggering factors.

    Alopecia treatment and management

  • There is limited success in the treatment of hairlessness. Medications like finasteride, dutasteride and minoxidil have been found to effective in preventing further loss of hair and with limited success in stimulating regrowth. Periodical corticosteroid injections are also given in the scalp to prevent the progression of the condition.

    Minoxidil, a nonprescription topical medication, is found to effective in treating male and female pattern baldness. Finasteride is an oral medication to be taken daily to get results. With the discontinuance of medication the loss may resume. There are many side effects. Expensive procedures like follicle transplants and hair transplants can be done with great success.
