What is lanugo hair?
Lanugo is the fine, soft and downy hair found on the body of a fetus or newborn baby.
The word lanugo is derived from the Latin word "lana", meaning wool. It is the first hair produced by the fetal hair follicles at about five months of gestation. Lanugo may also appear as a congenital defect in a baby or as an after effect of eating disorders.

The development of fetal lanugo

During fetal development, ectoderm forms the surface epidermis and contributes to different associated skin structures like hair follicles, nails and glands. By the forth month of gestation, cords of ectoderm cells extend into mesoderm forming epithelial columns. These cells give rise to hair follicles, sebaceous and sweat glands. At the same time the melanocyte stem cells, which have neural crest origin move into the base of the follicle forming cord.

During fifth month of gestation, lanugo growth gets initiated at the base of the cord and the lateral outgrowths of the cord give rise to sebaceous glands. The sebaceous glands start producing vernix (vernix caseosa).

Other ectoderm cords elongate and coil to form the sweat glands. By the sixth month of gestation, the body is fully covered by lanugo and has considerable deposit of vernix caseosa.

The lanugo growth over the entire body is at the same rate and the hairs appear to be of the same length. By the eighth month of gestation, the fetal skin is completely coated with vernix and the lanugo starts disappearing. By the ninth month of pregnancy, most of the embryonic hair is shed. Premature babies born before the complete shedding are seen still covered with embryonic hair.

Sometimes in full term babies also the lanugo is seen which may eventually disappears of its own accord within a few days or weeks. The persisting fine downy hair is completely shed when the baby is three to four months old. It is usually replaced by short, soft and near transparent vellus. It is also interesting to note that babies nearing their term in the womb swallow the lanugo along with amniotic fluid. The meconium (first stool) from newborn babies contains lanugo.

Function of fetal lanugo

Vernix caseosa is the waxy, cheese-like substance secreted by the fetal sebaceous glands to cover the fetal skin. Its secretion starts around 18 weeks into pregnancy and is seen coating the newborn babies. Vernix functions as a protective coating against the watery environment. Lanugo has the important role of binding the vernix to fetal skin.

Lanugo and eating disorders

Lanugo is observed in patients suffering from malnutrition and eating disorders like anorexia nervosa. Anorexia nervosa is characterized by a fierce quest for thinness in addition to having an intense fear of gaining weight. Most of the anorexia nervosa patients are adolescents, especially girls. Excessive preoccupation with body shape, self-image, perfectionism and negative self-evaluation leads to unregulated dieting, weight loss and malnutrition.

These patients require medical, nutritional and psychiatric support. Anorexia nervosa patients are known to grow fine, downy lanugo on the sides of the face, arms, and back. The condition differentiated from hirsutism, as in hirsutism only terminal hair is involved. Completely treating the anorexia nervosa can resolve the skin condition.

Lanugo in malignancy

Acquired lanugo-type hypertrichosis or hypertrichosis lanuginosa acquisita is sometimes associated with malignancy. Some patients affected by cancers have been observed to have such manifestations, though they may be lanugo-free as babies. The study by Slee PH et al concluded that malignancy associated lanugo is predominant in female patients in the age group of forty years to seventy years. It is observed in patients with colorectal cancer, lung cancer and breast cancer. They stressed that "the appearance of lanugo-type hypertrichosis in body areas previously perceived by patients as 'hairless' is highly indicative of internal malignancy."

Congenital lanugo in babies?

Hypertrichosis lanuginosa congenita, congenital lanugo-type hypertrichosis, is a rare skin disorder seen in some newborn babies. Vibhu Mendiratta et al in their case study on "Hypertrichosis lanuginosa congenita" reported about a three months old male baby. Babies born with this autosomal dominant cutaneous disorder, present lanugo hair covering the entire body. The palms, soles of the feet and mucous membranes are spared.

In these babies the hypertrichosis condition may increase or decrease with age. Lanugo babies may continue to have this type of hypertrichosis even after puberty. Hypertrichosis lanuginosa congenita may be associated with several other congenital defects in babies. In this particular case the baby was without any other manifestations of congenital defects.

What are Salmon Patches? - Nevus simplex Birthmarks on Babies

Salmon patches (nevus simplex) are very common vascular birthmarks found in about 30% of newborns.
These are congenital capillary malformations caused by dilated and stretched capillaries under the skin. Medically they are termed nevus simplex or n. flammeus nuchae. Salmon patches are also known as 'stork bite' when they occur at the nape of the neck and as 'angel's kiss' when they occur on the forehead, around eyes or between the eyebrows.

Salmon patches - Signs and symptoms

Nevus simplex is usually present at birth and in a few cases it may appear in the early weeks of life. Though these birthmarks may appear anywhere on the body, they are usually found on the back of the neck, forehead, on or around eyelids, on or around nose and upper lip.

Salmon patches - Diagnosis

Nevus simplex are easy to diagnose by visual observation. Salmon patches are pink in color and appear flat. They are irregularly shaped and may appear as small lesions or may cover a great area. During the flushing of the face due to change in temperature or crying of the baby these marks turn dark red. When pressure is applied on them, these birthmarks fade and appear pale.

Differential diagnosis of salmon patches

Nevus simplex can be differentiated from birthmarks like hemangiomas and port-wine stains. Skin hemangiomas have a rapid growth phase followed by an involution phase. They appear as raised swellings. Port-wine stains, may initially may have similar appearance, but do not fade and persist. Over time, port-wine stains become dark red or purple in color, thicken, become uneven and show hypertrophy (increased tissue mass). Nevus simplex are flat and do not grow.

Treatment of salmon patches

These birthmarks are temporary and in most of the cases they fade and disappear by two years. Normally nevus simplex do not require treatment as they disappear with the growth of the baby. If these birthmarks persist beyond three years, usually in the back of neck, they can be removed by cosmetic laser procedure to improve the aesthetic appearance of the baby.  There is no known preventive treatment for these birthmarks.

Prognosis
Most of these birthmarks fade and disappear in two years. Nevus simplex on the nape of the neck may not resolve and may persist. However salmon patches must be examined by pediatrician to rule out any complications or disorders.

Keratin (from dead epidermal cells) causes milia (small epidermoid cysts), when it gets trapped in small pockets on the epidermal surface as cysts/bumps, instead of getting exfoliated (sloughed off) naturally.
The symptoms of milia milk spots are that these small white bumps have no mouth (opening) and on abrasion redden on the edges leaving the central portion white.
Causes of milia in newborn babies
  • This mild skin disorder of baby milk spots or milia show the signs and symptoms as small bead-like raised white bumps on the skin around nose, ears, eyes, cheeks and chin of newborn babies.
  • In some babies these show symptoms like small bumps on the roof of the mouth, gums or genitalia.
  • Milia is very common in newborn babies and its signs and symptoms are found in almost half of them. They usually appear 3-4 days after birth of babies and last for about 20-30 days and resolve naturally.
  • In infants born prematurely the affliction is less and these oil seeds form late (after 2-3 weeks).
  • In rare cases rough bedlinen and clothing has been found to induce the formation of this ailment.

Causes of milia in adults
  • Milk spots can also appear on adult body more often on the face with unknown reasons.
  • Injured or inflamed epidermal region of the body also can form milia in some individuals due to set back in the skin exfoliating process.
  • Among the common and less understood causes of milk spots are the cosmetics and hair care products containing chemicals.
  • Facial creams sometimes smother our skin, preventing sloughing of keratin , dead epidermal cells and sebaceous matter.
  • Comedogenic nature of some facial make-ups, moisturizers and sunscreens (contrary to their claims of being non-comedogenic) causes milia, especially on the eye lids.
  • Some lip balms and lip sticks may be the causes of milk spots on the edge of lips vermilion border.
  • Regular exposure to sun and aging also leads to this skin condition.
  • Certain diseases like PCT (Porphyria Cutanea Tarda ) and epidermolysis bullosa can give rise to these bumps as the secondary disorder.
  • Skin stress and trauma due to radiotherapy and dermabrasion and the use of potent topical corticosteroid, develop symptoms of milia in some persons.
Genetic familial dominant autosomal inheritance disorders have been found to be one of the causes of milia.

Milk spots also called milia, oil seeds or baby milk spots are benign cysts filled with keratin.
They appear as small whitish or slightly pinkish bumps on the skin.
The top most layers of skin die and exfoliate. Milia are actually dead skin trapped as small bumps in pockets on the skin without natural exfoliation.
They also arises in undeveloped sebaceous glands. No opening is visible in it.
They appear usually on the skin around eyes, forehead, cheeks, chin and nose; sometimes they may appear on the genitalia.
Baby milk spots may also appear on gums and the roof of infants mouth (Epstein's pearls).
Baby spots are usually associated with newborns and infants.
About half of the newborn babies develop these. Both boys and girls are affected equally.
In young children baby spots usually disappear within 15-30 days. In infants no treatment is necessary and there are no lasting effects.
However this can affect persons of any age. In older children and adults they may persist and may require removal.
In adults who are very particular about looks, these can be removed.
Removal by an aesthetician or dermatologist is advised.
Secondary oil seeds have been found to occur in persons with sensitive skin stressed by harsh chemicals and heat.
They can also occur on inflamed or injured parts of the body.
They can also arise after a treatment with topical corticosteroids.

It is believed that the milia in some cases have hereditary predisposition and reports of dominant autosomal inheritance of this familial disorder have been documented.
